       <p>Highlights of a fetal laser surgery for twin-to-twin transfusion syndrome (TTTS) will be shown from Tampa General Hospital

TTTS affects 10 to 15 percent of identical-twin pregnancies and is the result of abnormal blood exchange between identical twins through a common placenta. The larger of the twins, or recipient, is surrounded by too much amniotic fluid and in danger of heart failure as its body tries to pump the overwhelming volume of blood intended for both. The smaller, or donor twin, is encased in a shrinking amniotic sac deprived of blood. Without treatment, both will likely die.

  <title>Bad hair surgery outcome: FUE - BHT repair of the worst case</title>

       <p>Dr Sanusi Umar repairs the worst case of bad hair surgical outcomes from prior procedures a patient underwent at other clinics. Some experts have considered this case the most extreme of poor hair surgical outcomes of note today. Many would understandably consider the patient's prior condition as evidence of botched surgeries and even surgical disasters. These procedures were commonly performed decades ago by clinics of the day when better options for hair restorations were yet to be developed. Disfigurement and scarring resulting from these antiquated techniques have remained a major source of anguish for many affected patients who have had to live secluded lives under hats and creative hairpieces for fear of ridicule. Body hair to head transplantation ( Body hair transplant ) has offered a ray of hope for some of these patients who are often out of options having depleted their traditional head donor supplies. 

The procedure is commonly called Body Hair Transplantation (BHT). Dr Umar uses BHT in patients that have a relative or absolute lack of traditional donor sources. In this case, the patient had lost his head donor after multiple antiquated scalp surgeries at other clinics, which left him still very bald with many disfiguring scars. The procedures incuded multiple scalp reductions, strip and punch hair transplants with the creation of unnatural plugs in the frontal scalp and hairline. He had ridging and pitting from these pluggy transplants. His temples and the half of his head was devoid of hair from progression of hairloss as well as the displacement of the area to the vertex by excessive scalp reduction. The crown had been deformed to form a slot with abnormally angled hair fanning out from it. Before seeing Dr Umar he underwent a double flap procedure at another clinic to correct for the poorly angled hair in his back. Having run out of traditional head donor hair, he was left with no hair transplant repair option. He went to see Dr Umar about using body hair as his only resort. Dr Umar carried out the repair using about 15000 chest, stomach, shoulders hair grafts, 6000 beard and only about 1000 head hair grafts. The procedure was completed in 3 major sessions at 6 monthly intervals. The result shown at the video was recorded at only 6 months after the last repair session. With yet more improvement to follow in the ensuing 9-12 months, the patient's life is already transformed from the progress made thus far to that of fulfillment and confidence. As the patient states, &quot;I have gotten my smile back&quot;...

The technique by which Dr Umar performs BHT is called Single Follicle Extraction and Transfer (SFET). This is his version of the procedure commonly referred to as Follicular Unit Extraction (FUE). The procedure is performed at his Redondo Beach office in California. DermHair Clinic is a division of FineTouch Dermatology Inc. Dr Umar is a fellowship trained cosmetic surgeon, and Certified dually by the American Boards of Dermatology and Internal Medicine.</p><p></p>

Targeting HIV replication

The replication of HIV 1 is a multi-stage process.

Each step is crucial to successful replication and is therefore a potential target of antiretroviral drugs.

Step one is the infection of a suitable host-cell, such as a CD4-positive T-lymphocyte.

Entry of HIV into the cell requires the presence of certain receptors on the cell surface, CD4 -- receptors and co-receptors such as CCR5 or CXCR4.

These receptors interact with protein-complexes, which are embedded in the viral envelope.

These complexes are composed of two glycoproteins:

an extracellular gp 120 and
a transmembrane gp 41

When HIV approaches the target cell gp120 binds to the CD4-receptors. This process is termed attachment.

It promotes further binding to a co-receptor. Co-receptor binding results in a conformational change in gp120.

This allows gp41 to unfold and insert its hydrophobic terminus into the cell membrane.

Gp 41 then folds back on itself.

This draws the virus towards the cell and facilitates the fusion of their membranes.

The viral nucleocapsid enters the host cell and breaks open releasing two viral RNA-strands and 3 essential replication enzymes:

Integrase, Protease and Reverse Transcriptase.

Reverse Transcriptase begins the reverse transcription of viral RNA.

It has two catalytic domains:

The Ribonuclease-H active site

And the polymerase active site

Here single stranded viral RNA is transcribed into an RNA-DNA double helix. Ribonuclease- H breaks down the RNA.

The polymerase then completes the remaining DNA-strand to form a DNA -- double helix.

Now Integrase goes into action.

It cleaves a dinucleotide from each 3-prime end of the DNA creating two sticky ends.

Integrase then transfers the DNA into the cell nucleus and facilitates its integration into the host cell genome.

The host cell genome now contains the genetic information of HIV.

Activation of the cell induces transcription of proviral DNA into messenger RNA.

The viral messenger RNA migrates into the cytoplasm where building blocks for a new virus are synthesised.

Some of them have to be processed by the viral protease.

Protease cleaves longer proteins into smaller core proteins.

This step is crucial to create an infectious virus.

Two viral RNA-strands and the replication enzymes then come together and core proteins assemble around them forming the capsid.

This immature particle leaves the cell acquiring a new envelope of host and viral proteins.

The virus matures and becomes ready to infect other cells.

HIV replicates billions of times per day destroying the hosts` immune cells and eventually causing disease progression.

Drugs which interfere with the key steps of viral replication can stop this fatal process.

Entry into the host cell can be blocked by fusion inhibitors for example.

Inhibition of reverse transcriptase by nucleoside inhibitors or by non-nucleoside Reverse Transcriptase- inhibitors is part of standard antiretroviral regimens.

The action of Integrase can be blocked.

Protease inhibitors are also part of standard antiretroviral therapy.

       <p>Your body's immune system has several defensive ploys to attack and destroy unwelcome and potentially harmful microscopic invaders.  

The first defense is the skin and lining of organs that connect with the outside.  These contain chemicals that break down unwelcome organisms.  

If an organism somehow sneaks past the skin barrier, your immune system then mobilizes a host of other defenses.  

Some cells surround the organisms while other cells, usually white blood cells called neutrophils, leave the blood vessel and enter the site of injury or infection.

Now, other bacteria-eating cells called macrophages, travel to the injured site.  

They extend tentacle-like arms that reach out and snare the invading organism.  The macrophage then destroys the microbe with powerful chemicals or swallows it whole.

Macrophages, neutrophils, and skin, along with other immune components such as antibodies, provide a multi-faceted defense against infection.</p><p></p>

The notion that ovarian cancer is a &quot;silent killer&quot; without early warning signs has changed. 

The Gynecologic Cancer Foundation has announced a national consensus on symptoms for ovarian cancer. Symptoms that women should consider as warning signs include: bloating, pelvic or abdominal pain, difficulty eating or feeling full quickly, and frequent or urgent urination. If women have these symptoms daily over the course of several weeks they should consult with a gynecologist about them, particularly if the symptoms are new or unusual.
