She was deeply hypotensive and she had Atrial fibrillation , TTE shows severely impaired left ventricular sistolic function , no dilatation but mild hypertrophy . Anterior mitral leaflet has a scarce early diastolic opening and CFM shows +++ mitral regurgitation ( Class I Carperter ) - Additional findings : Aortic insufficiency and aortic sclerosis . PW on Mitral shows a pattern of diastolic impairment . Dobutamine has been promptly started at 8 mcg/Kg/ min . She was under mechanical ventilation because of the occurrence of acute Respiratory failure .
